Methylone, a cathinone derivative and beta-keto analog of methylenedioxymethamphetamine (MDMA), is an entactogen and stimulant marketed as bath salts, plant food, research chemicals, or as a replacement for MDMA in “Molly” capsules. It is appealing to users because it produces euphoria and increased sociability; however, it can be extremely dangerous as it can also cause paranoia, agitations, hallucinations, and psychotic behavior. The Miami-Dade Medical Examiner Department (MDME) has investigated 72 cases since 2011, in which methylone was identified by the toxicology laboratory during a routine drug screen. Of the death investigation cases received over the last 6 years, 46 were classified as homicides, 8 were classified as suicides (gunshot wounds and hangings), 16 were classified as accidents, and 2 were classified as undetermined. Of the accidental deaths, seven were overdoses; however, only three of the accidental deaths were directly related to the ingestion of methylone and its toxic effects.
